• Unit Charter Application

 This form is required to be completed by all units of the WWII HRS Inc.

• Change of Command Form

 This form is required when the unit commander changes for any reason.

• Event Request Form
  
    All event coordinators are required to complete these forms in
  order for their events to have HRS insurance coverage.

• Certificate of Insurance Request

This form is a request for a copy of the Certificate of Insurance document requested by most event organizers to prove that the WWII HRS has limited liability insurance coverage.

 

NOTE: Hosting units need to ask if the facility needs a "Certificate of Insurance" or "Proof of Insurance."  CoI's may require a fee under our new insurance policy. Allow 2-4 weeks for processing.

• Event Waiver Form  (WWII HRS Member)

This form is required for all HRS members at HRS events.  All completed forms must be sent to the Secretary within 30 days after the event.

• Event Waiver Form  (Non-HRS Participant) 

This form is required for all Non-HRS members that attend HRS events.  All completed forms must be sent to the Secretary within 30 days after the event.

• Parental Consent Form

     This form is required for all HRS members that are under the age of eighteen.
